Transition from the paediatric to adult epilepsy teams happens at the most appropriate point between the ages of 16-19 years for the individual concerned.
The process starts with a formal referral from the paediatrician to the adult neurologist. However, prior to the care being formally handed-over to the adult team, the young person and their parents/carers are invited by the children’s epilepsy nurse to meet with one of the adult epilepsy nurses in a transition clinic.
